Applications are invited from enthusiastic individuals to fill the role of Band 5 Community Nurse within Adult Community Services in the Thanet locality.

The post holders will work as members of our diverse and friendly Community Nursing Team. Your responsibilities will include visiting patients in their own homes as well as residential care homes to provide a comprehensive range of clinical nursing interventions including administration of IV drugs, insulin administration, complex wound management, health promotion and medicines management, end of life care and symptom control. As a Community Nurse you will need to demonstrate a good level of clinical skills or be willing to learn and develop existing skills. A high standard of holistic assessment and care planning is also expected.

Our community nurses work closely with members of the wider multi-disciplinary teams, including our GP’s, local Hospices, Health and Social Care Coordinators, Intermediate care Teams, Specialist nurses and Therapist colleagues. All teams within the locality work as integrated teams with the use of MDTs.
