An opportunity has arisen to increase the current establishment of the Senior Nurse Cluster Lead team, within the West Kent locality of Kent Community Health Trust.

The Cluster Lead Band 7 Senior Nurse is responsible for line management and support to aligned Team Leaders, and support proactive management of their caseloads, whilst developing staff with clinical skills. The Cluster Lead will offer experience and guidance to manage the more complex patients and the role includes teaching clinical skills, assisting with competency sign off and ensuring standards are met.

This role provides assurance of our quality agenda, and allows the opportunity for some management experience, whilst retaining a clinical leadership role.

We are looking for an experienced community Team Leader, or senior clinician looking to further their skills and career. The successful applicant will support the community Team Leaders to deliver a high standard of care, working closely with the Complex Care Nurses, Other Specialist Services, GP's, Social Services, Hospice and other MDT partners to provide an integrated approach caring for patients in their own homes.

Applicants will have the specialist practitioner community degree or equivalent, and should be experienced within the Community setting.

They will support the teams within the clusters to provide high quality care ensuring that the Patients’ best interests are at the centre of service delivery.

The Cluster Lead will focus on enhancing the quality of life for people with long-term conditions, helping people to recover from episodes of ill health and caring for those requiring End of Life care.

The ideal candidate for this role will be have excellent communication skills, be passionate about empowering patients, collaborate with others to deliver high quality care along a care pathway and across organisational boundaries. They will be innovative in how care can be delivered in the future. They will also be prepared to undertake additional education and training to support professional development.

This is a full time post however 30 hours will be considered.
