The Herne Bay Intermediate Care Team (ICT) provides a holistic service to our patients, including emergency rapid response care alongside therapy and nursing input. The team covers a wide geographical area and you must be able to travel in a timely manner between visits.

You will be working as part of the ICT inter-disciplinary Rehab Team providing assessment and intensive rehabilitation for adult/ elderly patients in the community. Your case load will include those referred by GP, local referral unit with the history of falls, mobility problems, musculoskeletal problems and some Neuro related problems. You may also occasionally be required to provide physiotherapy assessment and treatments to support the wider team.

You will have experience working with patients with various medical problems. You will have excellent clinical assessment and treatment skills, be able to utilise excellent communication skills and be passionate about empowering patients. You will need to work collaboratively with others to deliver high quality care across organisational boundaries, being innovative in how we can deliver care in the future.

In line with government and commissioning agendas, Intermediate Care has developed to meet the diverse and changing needs of the local population. Being part of a large inter-disciplinary team allows close collaborative working and opportunities for joint professional training. Clear, established supervision and appraisal structures are in place. Although the role involves lone working, access to senior clinical support is available.

This post is a fixed term contract until 15.12.18.
