A Therapy Lead is required to work for the Integrated Discharge Team (IDT) to work across the William Harvey Hospital, Ashford and the Kent and Canterbury Hospital, Canterbury.

The role is to work within the A&E department and Urgent Care, Leading the Therapy team within IDT to assess patients and plan their safe discharge from hospital within a timely manner.  To ensure that Therapy is covered over 7 days between 8am-8pm.

The IDT is a multi-professional, multi-organisational team, working in partnership with both acute and community-based service providers, to support the safe, efficient and effective discharge of patients from an acute hospital bed, seven days a week. By actively promoting the principle of ‘Home First’, the post-holder(s) will actively encourage patients to regain / maintain their independence, enabling safe discharge home or to a short term placement.

As stated, the IDT is an integrated team, which includes staff employed by:
• East Kent Hospitals University Foundation Trust (EKHUFT)
• Kent Community Health Foundation Trust (KCHFT)
• Kent County Council
