We are currently looking for a highly motivated and passionate Support worker to join our multi-disciplinary Ashford Intermediate Care Team (ICT). The team covers Ashford and Tenterden locality; the successful applicant would be expected to work across these areas. The post holder will provide short periods of intervention to clients in their own home environment, facilitating clients to achieve maximum potential following periods of illness or injury through providing rehabilitation, personal care and meal preparation.

They will work as part of an inter-disciplinary team carrying out rehabilitation and care work. The post involves shift work and the service operates 7 days per week. The role will involve working with patients and other health care professionals to provide a holistic approach to patient care, thereby facilitating early discharge from hospital, prevent hospital admissions and aid patients to stay independent in their own home for as long as possible.

The successful candidates must be able to travel across a widespread geographical area within a shift to visit patients in their own home.
